It is a requirement of our online training that you must complete an authenticity declaration stating that you completed the assessment yourself that will be provided to you following successful completion of your assessment.
The completed declaration must be returned to the assessment team by email, fax or post prior to issuance of your training certificate.
Authorised Person
The declaration must be signed by an authorised person (see list of 51 persons below)
Proof of Identity
You must provide a copy of your photo proof your identity (e.g. driver licence, passport, proof of age card) that has been sighted and signed by the authorised person.
